Job summary

AdventHealth seeks a dedicated Care Manager to support discharge planning and post-acute care coordination at our Orlando facilities. This role emphasizes holistic patient assessment, multidisciplinary teamwork, and timely care transitions.

The position covers full-time nights, about 36 hours per week, with duties including advocacy and ongoing collaboration with families and care teams at AdventHealth in Orlando, FL.

Qualifications

• Master's degree required.
• 2+ care management experience preferred.
• 2+ social work experience required.

Responsibilities

• Provides grief counseling, disease adjustment support and crisis intervention.
• Assesses discharge planning needs for inpatient/ED patients to avoid delays.
• Reviews medical records and incorporates factors into transition of care plan.
• Develops discharge plans and coordinates post-acute services and facilities.
• Communicates with care teams and Payors for post-acute authorization.
• Organizes care conferences and documents evaluations, barriers and patient needs.
