AdventHealth in Kissimmee, Florida, is hiring for a Care Management/Social Work role to support discharge planning across inpatient, observation, and emergency departments. You will assess needs, coordinate services, and provide crisis intervention and family support to ensure timely progression of care.

Ideal candidates hold a Master’s in Social Work with 2+ years in care management and social work. This full-time, day-shift position offers career development and comprehensive benefits.

Competenze

• Master's degree in Social Work or related field.
• 2+ years of care management experience preferred.
• 2+ years of social work experience required.

Mansioni

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning, and de-escalation services for patients and families.
• Assesses patients’ and families’ wholistically for discharge planning needs in inpatient, observation and/or emergency departments.
• Develops discharge plans with appropriate contingency plans throughout the hospital stay and coordinates post-acute care services.
