Job summary

AdventHealth is seeking a discharge planning professional to provide holistic care coordination across inpatient, observation, and emergency departments. You will assess medical, psychosocial, and financial factors to develop discharge plans and coordinate post-acute care arrangements.

The role requires a Master’s degree, 2+ years in social work, and strong communication with patients, families, and care teams. You will work within a faith-based health system dedicated to Whole Person Well-being.

Qualifications

• Master's degree required.
• 2+ social work experience required.
• 2+ care management experience preferred.
• Knowledge of discharge planning processes and post-acute care resources.

Responsibilities

• Provides discharge planning, crisis intervention and coordination of care for inpatient, observation and ED patients.
• Develops discharge plans with contingencies for post-acute care services and community resources.
• Communicates with post-acute care services and families to ensure continuity of care and accurate medical records.
• Organizes and facilitates patient and family care conferences with the multidisciplinary team.
• Documents discharge planning evaluations, barriers to progression, and rationale for discharge decisions.
