Job summary

Denver Health is hiring a Social Worker MSW for the Adolescent Inpatient Unit. The role focuses on planning and delivering social work treatment for ED/hospitalized patients and families, with emphasis on biopsychosocial assessment, discharge planning, and care coordination.

You will provide crisis intervention, counseling, and coordinate with a broad network of health and community resources, ensuring patient-centered care and continuity across care settings.

Qualifications

• Master's degree in Social Work from CSWE-accredited program required.
• 1–3 years post-master's social work experience preferred.
• Knowledge of care coordination, crisis intervention and discharge planning.
• Ability to engage patients across the lifespan with cultural sensitivity.

Responsibilities

• Plan, organize and implement social work treatment for ED/hospitalized patients and families.
• Assess biopsychosocial needs and create individual care plans for health concerns.
• Provide crisis intervention, supportive counseling, case consultation and care coordination.
• Coordinate with internal/external resources to maximize patient outcomes.
• Facilitate multidisciplinary meetings and ensure patient involvement in care planning.
