Acadia Healthcare seeks a Social Worker Inpatient to provide psychosocial assessment, crisis intervention and comprehensive discharge planning. This full-time role requires 8a-430p shifts with weekend rotation and state licensure (LMSW/LCSW/LSW/APSW).

CPR and de-escalation certification are required, with opportunities for professional development. You will work within a multidisciplinary team, coordinate patient cases, lead education groups, and ensure accurate documentation while supporting

Competenze

• Master's degree in Social Work is required.
• Two or more years’ experience with the patient population preferred.
• State licensure (LMSW, LCSW, or LSW) and CPR/de-escalation certification required.

Mansioni

• Conduct psychosocial assessments and develop effective treatment plans.
• Obtain patient needs and limitations to implement treatment plans.
• Provide crisis intervention and assist families in understanding implications.
• Participate in multidisciplinary team meetings for discharge planning.
• Identify, assess, and coordinate patient cases and services.
• Facilitate education, process groups, and referrals as requested.
• May conduct intakes and coordinate placements within facility programs.
• Promote recovery and highest level of social functioning.
• Communicate with family members to support the patient.
• Manage caseload and maintain patient chart documentation.
• Ensure documentation meets facility and accreditation standards.
