SolutionHealth is seeking an Inpatient Behavioral Health Social Worker Case Manager to support adult patients with acute and chronic psychiatric disorders. You will assess, plan, and coordinate care to optimize outcomes and discharge planning within a hospital setting.

The role emphasizes collaboration with the health care team, documentation, and utilization management to ensure high quality, cost-effective care for patients and families.

Qualifications

• Requires MSW degree and NH LICSW preferred.
• 1–3 years of experience in a health care system, preferably in a hospital setting.
• Excellent clinical documentation; EMR experience preferred.

Responsibilities

• Collaborates with the primary nurse and multidisciplinary team for ongoing social work assessment.
• Participates in development and evaluation of integrated clinical pathways.
• Provides supportive counseling and crisis intervention for patient and family as needed.
• Monitors patient progress to ensure timely care.
• Acts as a resource to staff for discharge planning and psychosocial problem resolution.
• Assesses patient/family adaptation to illness and capacity to provide care via biopsychosocial assessment.
• Serves as liaison to community social service agencies for discharge options.
