Community Healthcare Network, Inc. in New York City seeks a social worker to assess adolescents and connect them with essential services. You will collaborate with families, schools, and healthcare teams to strengthen life skills and support health outcomes.

Responsibilities include conducting psychosocial assessments, providing short-term counseling, documenting in medical records, and coordinating referrals while maintaining confidentiality in a community-focused, not-for-profit setting.

Formación

• Masters in Social Work required.
• Licensed Master of Social Work (LMSW/LCSW) required.
• Experience with adolescent social assessment and counseling preferred.

Responsabilidades

• Assess adolescents' social and emotional needs and develop a plan with the client.
• Provide services to the client and family or assist them in securing such services.
• Record data in the client’s medical record concerning services given.
• Provide short-term goal-oriented social work interventions; carry a minimum of six cases for short-term counseling.
• Identify outside resources for referrals and establish working relationships; monitor contracted services.
• Conduct HIV universal counseling, including pre- and post-test counseling, as needed.
• Participate in prenatal, family planning and primary care programs, including psychosocial assessment for all patients.
• Provide health education services to individuals and groups.
• Participate in staff development opportunities.
• Facilitate related services for health center patients and CHN staff with confidentiality and privacy.
• Serve on committees or task forces; co-facilitate group work for special needs populations.
