Job summary

University of California, San Francisco seeks a Clinical Case Manager for MAJIC/Intake to provide culturally responsive, trauma-informed counseling, assessments and case management to patients at Ward 93 and justice-involved individuals in the MAJIC program. The role supports MOUD linkage and coordination of care across services.

Under supervision of the Intake & Counseling Services Director, you will use evidence-based models (CBT, motivational interviewing) and coordinate resources to promote

Qualifications

• Bachelor's degree in related area and / or equivalent experience / training.
• Requires clinical licensure in relevant discipline.
• Certified Substance Use Disorder/Alcohol Drug Counselor per CA DHCS standards.

Responsibilities

• Screens, triages and assesses individuals presenting for intake.
• Establishes and coordinates a supportive, person-centered care plan with goals and interventions.
• Provides evidence-based counseling services within scope of practice.
• Coordinates clinical and non-clinical resources based on diagnosis, insurance and programs.
• Assists with contingency management or motivational incentives based on goals.
