Job summary

Iron Wellness and Recovery Center in Trenton, NJ is seeking a Care Coordinator to support MOUD enrollment, intake, warm handoffs, and ongoing care coordination in a grant-funded program. The role emphasizes trauma-informed, recovery-oriented care and collaboration with clinical staff to ensure timely access to treatment.

You will help connect clients to housing, benefits, employment, and community resources while supporting in-person MOUD delivery and wraparound services.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Human Services, or a related field is required.
• Experience providing case management services in substance use or behavioral health settings.
• Familiarity with ASAM Criteria and levels of care.
• Preferred credentials include CADC, CADC-eligible, LCADC, or peer Recovery credentials.

Responsibilities

• Support enrollment, intake, warm handoffs, retention, and care coordination for MOUD clients.
• Provide intake screening, DSM-5-TR assessment support, and ASAM placement documentation.
• Coordinate services with healthcare providers, community resources, and VA/Medicaid services.
• Deliver contingency-management incentives and documentation.
• Assist clients with housing, benefits, employment, and recovery supports.
