Job summary

Mental Health Systems, Inc. in San Diego seeks a Clinical Case Manager to coordinate care for adults with mental health and substance use needs. You will provide case management, crisis interventions, and referrals across medical, educational, housing and social supports.

The role requires a master’s or bachelor’s in social services with relevant experience, a valid CA driver’s license, and the ability to work as part of a multidisciplinary team in a community-based setting.

Qualifications

• For Clinical CM, master's degree with two years of experience in social services; or bachelor's degree with four years of experience in social services.
• For SUD CM a certification from recognized board is preferred and registration required.
• Valid CA driver’s license and current auto insurance per contract requirements required.
• Experience providing mental health and/or substance use disorder services preferred.
• Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

• Provide case management with crisis intervention, discharge planning, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services.
• Facilitate placement including pre-placement visits, and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
