Job summary

Advantage Behavioral Health Systems seeks a Care Coordinator/Medical Assistant to join a multidisciplinary team serving individuals with OUD, SUD, and co-occurring mental health conditions. You will assist with intake, vital signs, MOUD logistics, and care coordination between the mobile unit and Clarke outpatient clinic.

Responsibilities include scheduling, outreach to reduce missed visits, and collaboration with clinicians to ensure continuity of care.

Qualifications

• High school diploma or GED required.
• Medical Assistant certification and healthcare experience preferred.
• Minimum of one year in healthcare, behavioral health, public health, or community health setting.
• Proficiency with EHR systems and Microsoft Office applications.
• Knowledge of MOUD, crisis prevention, and social services linkage is a plus.

Responsibilities

• Room clients and prepare them for provider visits.
• Obtain and document vital signs and screenings as per protocol.
• Schedule referrals, appointments, and follow-up visits.
• Document patient information in the EHR accurately.
• Coordinate care across medical, behavioral health, and social services.
