Mental Health Systems, Inc. is seeking a Justice-Enhanced Case Manager in San Diego, CA.

This regular full-time role over 30 hours supports program goals by coordinating access to medical, educational, social, vocational, and housing services, and by initiating referrals for next-level care. The role involves advocating for clients, coordinating with health providers, and assisting with navigating the justice system, including court appearances and coordination with probation and other agencies.

Qualifications

• Master's degree with two years of experience in social services, or bachelor's degree with four years of experience.
• Valid CA driver's license and current auto insurance per contract requirements.
• Experience providing mental health and/or substance use disorder services preferred.
• Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

• Provide case management with crisis intervention, discharge planning, group & individual work, and counseling.
• Complete accurate and timely documentation and meet minimum billable requirements.
• Facilitate placement and transportation as needed.
• Provide field-based services as required by program and client need.
• Maintain client confidentiality and participate in program meetings.
• Assist clients with navigating the justice system and liaising with probation, parole, jails, and other entities.
