Job summary

Concord Hospital Health System in New Hampshire is seeking a Rehabilitation Services support role to assist with mobilization and personal care under supervision. Qualifications

• High school diploma or GED required.
• Current American Heart Association Basic Life Support for Healthcare providers, or equivalent credentialed within 30 days of hire.
• None required.

Responsibilities

• Prepare and position patient before/after skilled care; assist with ambulation and positioning under supervision.
• Demonstrate safe patient handling and use of equipment; identify need for additional resources and seek assistance.
• Safely mobilize patients within bed and to chair/toilet; report concerns to nurse/therapist.
• Set up and take down mobility equipment (beds, lifts, walkers, canes, crutches).
• Document inpatient interventions in EMR and coordinate with nurse/therapist; obtain medical clearance.
• Complete department requirements and perform other duties as assigned.
