Job summary

180 Degrees, Inc. seeks a Mobile Case Manager to bridge shelter and community, supporting youth after leaving Brittany’s Place in St. Paul. You’ll operate in the field, ensuring continuity of care and coordination with shelter staff in a continuum-of-care model that follows youth into daily life.

You’ll work full-time on-site, with strong supervision and team collaboration, and a focus on safety, documenting progress and maintaining professional boundaries in a fast-paced environment.

Qualifications

• Bachelor’s degree in a human services field and two years of case management experience, or equivalent.
• Nonprofit sector experience preferred.
• Valid driver’s license with reliable transportation for community travel.
• Ability to clear a background screening and maintain confidentiality.

Responsibilities

• Serve as a consistent connection between shelter operations and the community.
• Provide prevention-focused, community-based case management to youth exiting shelter.
• Maintain structured aftercare to ensure continuity of care during transitions.
• Build outreach presence in spaces where youth gather.
• Collaborate with shelter staff and other Mobile Case Managers for program alignment.
• Work independently in the field while staying connected to Shelter supervision.
• Support planned and unplanned discharges with transition planning.
• Maintain professional boundaries, confidentiality and ethical practice.
• Keep clear, accurate documentation to support youth planning and progress.
