Job summary

Twin Cities Physicians is seeking a Nurse Practitioner to deliver on-site primary and wound care across senior living communities in Minnesota. This role blends geriatrics with wound care management, supporting chronic disease care and preventive strategies.

You will join a provider-led group focused on continuity of care, proactive intervention, and reducing hospitalizations. A strong preference for AGNP/CNP and Minnesota licensure with willingness to travel.

Qualifications

• Active Minnesota CNP license.
• Experience in geriatrics, primary care, post-acute care or related settings preferred.
• Wound care certification welcomed but not required.
• Valid driver's license and ability to travel across Twin Cities Metro.
• Passion for aging adult care and ongoing professional growth.

Responsibilities

• Provide on-site primary care and rounding in senior living communities.
• Manage chronic conditions, preventive care, medication reconciliation, diagnostics, and care plans.
• Deliver wound care services and participate in advanced wound management initiatives.
• Coordinate care with physicians, nursing staff, therapists, caregivers, and families.
• Support continuity of care while reducing avoidable hospitalizations and ER visits.
• Build expertise in both primary care and wound care management.
