Job summary

Powerback Rehabilitation is seeking a Speech Language Pathologist for part-time, mobile outpatient therapy across Nashua and surrounding New Hampshire locations. The role focuses on in-home and facility-based services for older adults and other patients, delivering individualized treatment plans in functional environments.

The ideal candidate holds a current SLP license and a valid driver's license, with the ability to travel between patients.

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice, or license eligibility.
• Must have a valid driver's license or reliable transportation to patients' residences.

Responsibilities

• Provide outpatient speech-language pathology services in home and facility settings.
• Evaluate patients and develop individualized treatment plans.
• Coordinate with interdisciplinary rehabilitation team for patient care.
• Document assessments, progress notes, and maintain accurate records.
