Job summary

South County Mental Health Center, Inc. is seeking a compassionate Family Systems Navigator to join our Mobile Response Team (MRT).

The role helps families involved with child welfare access services, coordinates care across systems, and serves as a liaison between MRT and community partners to close gaps in care. Ideal candidates hold a bachelor’s degree in a human services field and at least five years working with children’s systems, with strong knowledge of Child Welfare, School Systems, and

Qualifications

  • Bachelor’s degree in a human services field.
  • Minimum five years of experience working within children’s systems.
  • Strong knowledge of Child Welfare, School Systems, and Juvenile Justice.
  • Strong communication, collaboration, and organizational skills.
  • Ability to work effectively with families, system partners, and multidisciplinary teams.

Responsibilities

  • Assist families involved with child welfare in navigating the child welfare system.
  • Coordinate services across multiple community systems to reduce fragmentation of care.
  • Serve as a navigation resource for families and system partners.
  • Work with Child Welfare to prevent placement disruptions for foster families.
  • Develop and maintain collaborative relationships with system partners.
  • Attend system partner meetings and provide MRT representation when needed.
  • Conduct outreach to system partners to raise awareness of MRT services.
  • Provide education regarding the benefits and appropriate use of Mobile Response Services.
  • Support development of MOUs and Collaborative Agreements with partners.
