Job summary

Children’s Home Network seeks a Family Navigator-CSS in Clearwater, FL to deliver navigation services supporting families and children. The role emphasizes linkage to community resources, care coordination, and crisis intervention, with a focus on preventing child abuse and neglect.

You will document services, participate in Family Team Conferences, and collaborate with a multidisciplinary team to support youth and families, including plan development and referrals.

Qualifications

• Bachelor’s Degree required, with at least one year of experience working with children and families.
• Degree may be in education, health, social work, counseling, or related field.
• Understanding of parenting and child development.

Responsibilities

• Help families access community resources and training for healthy child development.
• Provide parenting support, education, and care coordination for at-risk caregivers.
• Monitor needs and implement intervention plans.
• Participate in Family Team Conferences and multidisciplinary discussions.
• Document services timely per program requirements.
• Coordinate parent meetings and workshops on child health and development.
• Maintain Caregiver Support Services program fidelity.
• Complete pre/post assessments to measure progress and plan closure.
