Job summary

Eliot Community Human Services seeks a Case Manager to provide respite services for adults with serious and persistent mental illnesses. You will deliver mobile and site-based case management, coordinating medical and community resources, and developing individualized action plans to support recovery.

Ideal candidates have a Bachelor’s or Master’s in Psychology, Social Work, or related fields, with at least 2 years of relevant experience and a MA driver’s license.

Qualifications

• Requires a Bachelor’s degree or Master’s degree in Psychology, Social Work or related field.
• At least 2 years of experience working with adults with mental illness.
• Valid Massachusetts driver’s license.

Responsibilities

• Provide intensive and mobile case management, care coordination and support to assigned consumers in the community or at Respite sites.
• Coordinate services including income, insurance, entitlements, and scheduling of medical/psychiatric appointments.
• Develop Individualized Action Plans with culturally competent, strength-based goals to support recovery.
• Advocate for clients and assist with IAP goals within the caseload.
