Job summary

Eliot Community Human Services in Massachusetts is seeking a Case Manager to join our Respite program, providing short-term, restorative case management for adults with serious and persistent mental illnesses. Qualifications

• Requires a Bachelor’s degree or Master’s degree in Psychology, Social Work or related field.
• At least 2 years of experience working with adults with mental illness.

Responsibilities

• Provide intensive and mobile case management, care coordination and support and interventions to assigned consumers.
• Coordinate and facilitate services including income, insurance, entitlements, and scheduling of medical and psychiatric appointments.
• Develop Individualized Action Plans (IAPs) for each consumer with recovery-focused goals.
• Advocate for assigned caseload and support IAP goals.

Eliot is seeking a Case Manager to join our team in providing Respite services to adults with serious and persistent mental illnesses. Eliot’s Respite program is a short term restorative service that assists clients with obtaining and maintaining community living. As a Case Manager, you will be responsible for providing case management and coordination for 8-12 consumers. Our ideal candidate will have the skills to establish supportive relationships, excellent communication, crisis management and problem solving skills.

Responsibilities:

• Provide intensive and mobile case management, care coordination and support and interventions to assigned consumers either in the community or at site-based Respite.
• Coordinate and facilitate services which may include assisting consumers with managing income, insurance, other entitlements, accessing other services and community resources, and scheduling/coordination of all medical and psychiatric appointments.
• Develop IAP’s (Individualized Action Plans) for each consumer as assigned. Ensure that treatment goals are culturally competent, strength-based, realistic, attainable, and promote recovery.
• Provide support for assigned caseload, including advocacy and assistance with IAP goals.
