Job summary

Eliot Community Human Services, Inc. is hiring a Case Manager to deliver Respite services for adults with serious mental illness. You will provide intensive case management and coordinate care in the community or at site-based Respite.

Ideal candidates hold a Bachelor’s or Master’s in psychology or social work and have at least 2 years of relevant experience. Access to supportive services and a collaborative team environment are available.

Qualifications

• Bachelor’s or Master’s degree in Psychology, Social Work or related field.
• At least 2 years of experience working with adults with mental illness.
• Strong case management, communication, and crisis-resolution skills.

Responsibilities

• Provide intensive case management and coordination for 8–12 adults in Respite.
• Coordinate services including income, insurance, entitlements, and appointments.
• Develop Individualized Action Plans focused on recovery and cultural relevance.
• Advocate for clients and support IAP goal achievement.
