Job summary

Ennoble Care in Volusia, FL is seeking an experienced Full-time Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to patients in the home.

Under physician supervision, you will assess, diagnose, and manage chronic conditions, coordinating with our in-house clinical team and caregivers for a patient-centered plan.

Qualifications

• Board Certified, with a valid NP/PA certification in the State of Florida.
• 1–2 years of primary care experience with geriatric patients.
• Hospice experience is preferred but not required.
• Strong clinical skills with the ability to assess, diagnose and treat confidently.

Responsibilities

• Provide high-quality primary and palliative care to patients in and around Volusia, FL.
• Perform physical examinations and review medical histories to develop care plans.
• Communicate effectively with office staff and clinical operations across internal platforms.
• Attend quarterly medical staff meetings and contribute to treatment planning.
