Job summary

Powerback Rehabilitation is seeking a Physical Therapist for full-time roles in Raynham, MA, with in-home outpatient and community-based caseloads, including ILF/ALF settings. The position offers a $5,000 sign-on bonus and a flexible salaried schedule suited to experienced clinicians.

The role requires active PT licensure, a Master’s degree (or equivalent), and a track record in home-health or outpatient rehab. A driver’s license and willingness to travel are essential.

Qualifications

• Must have a valid Physical Therapy license in the state of practice or proof of license eligibility.
• Master’s degree in Physical Therapy; or Master’s in PT with Bachelor’s in PT.
• Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• The qualifying PT degree earned outside the U.S. must be evaluated as U.S. equivalent.
• Must have a valid driver’s license or reliable transportation to/from patients’ residence.

Responsibilities

• Provide outpatient rehabilitation services in-home or in an independent living facility for older adults.
• Develop and deliver individualized PT programs to restore, reinforce, and enhance function.
• Educate and train patients and caregivers in skills for independence.
• Supervise PT aides, temporary licensees, students and support personnel per licensure.
• Maintain timely documentation and attend care conferences and related meetings.
• Refer to other disciplines as needed and collaborate with rehab team.
• Promote safe environment and adapt plans of care to patient needs.
• Order supplies and maintain equipment and sanitization.
