Job summary

Powerback Rehabilitation is seeking a Physical Therapist to deliver full-time outpatient rehabilitation in Raynham, MA and surrounding areas, with in-home visits possible. The role focuses on older adults in ALF/ILF and community settings, delivering therapy across functional domains.

You will assess, plan, and implement customized PT programs, supervise assistants where applicable, and ensure timely documentation.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or an equivalent combination.
• Bachelor's degree in Physical Therapy with 5 years of PT experience (as applicable).
• Valid driver’s license or reliable transportation.

Responsibilities

• Provide outpatient rehabilitation services in home and community settings.
• Develop and deliver individualized PT programs and educate patients and caregivers.
• Supervise PT Aides, temporary licensees, students and support staff as needed.
• Complete timely documentation and adhere to scheduling and billing requirements.
• Incorporate health and wellness education and community mobility assessments into care.
