Job summary

Rehab Without Walls Neuro Rehabilitation is seeking an experienced Physical Therapist to deliver functional rehabilitation in patients’ homes and community settings across Georgia. You will treat clients in real-life environments, not just staged clinics, enabling independence and safety in daily activities.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current BLS/CPR certification per state regulations.
• Minimum of one year as a physical therapist preferred.
• Knowledge of rehabilitation techniques related to complex neurological injury.
• Communicates effectively and professionally in verbal and written interactions.
• Ability to lift 50 pounds; may lift up to 100 pounds following safety procedures.
• Duties require standing, walking and manual handling for extended periods.

Responsibilities

• Perform evaluations and develop treatment plans
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences
• Perform discharge evaluations and provide home/community safety recommendations and resources
• Communicate patient needs and progress to the treatment team, physician, patient and family
• Coordinate with other disciplines to ensure collaboration and optimize patient outcomes
