Job summary

Powerback Rehabilitation is seeking a licensed Physical Therapist for mobile in-home outpatient therapy in East Longmeadow, MA. This full-time role includes a $5,000 sign-on bonus and a comprehensive benefits package.

The PT will evaluate and treat older adults in home settings or living facilities, develop individualized plans of care, educate patients and families, and collaborate with a multidisciplinary team to restore function and promote independence.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master's degree in Physical Therapy; or Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or Bachelor's degree in Physical Therapy plus 5 years of PT experience.
• The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
• Must have a valid driver's license or reliable, independent transportation to and from the patients' residence.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students and support personnel as needed and per state licensure.
• Adhere to department and patient treatment schedule, and complete timely clinical and billing documentation.
• Attend and contribute to patient care, staffing conferences and related meetings.
• Make referrals to other disciplines and agencies to address patient needs.
• Consult with rehab team to select appropriate therapy regimen and adapt Plan of Care.
• Promote safe environment and enforce safety measures.
• Order supplies and equipment as necessary.
• Maintain and report equipment repairs and upkeep.
• Provide patient-centered care with high quality service and compassionate approach.
• Screen, examine, and evaluate patients to establish diagnosis and treatment plans.
