Qualifications

• Valid Physical Therapy license in state of practice or license eligibility.
• Must have driver's license or reliable transportation to patients' homes.

Responsibilities

• Provide outpatient rehabilitation in the patient’s home or in an ILF/ALF setting.
• Develop and deliver individualized PT programs to restore function.
• Coordinate with rehab team and refer to other disciplines as needed.
• Supervise PT Assistants and support staff as required by licensure.
