Job summary

PHC is seeking an Occupational Therapist to provide patient care in Gaston and Lincoln Counties, including Denver, NC, with a focus on improving daily living and work skills. The role involves travel to client homes and SNF/ALF settings, delivering start-of-care assessments, therapy sessions, and caregiver education.

The position offers flexible scheduling, competitive pay, and one-on-one training in a fast-paced home health environment. Part-time/PRN hours with on-the-road duties.

Qualifications

• Active NC or multi-state OT license.
• Current CPR certificate.
• 2–3 years home health experience preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and in a team.
• Strong problem-solving and critical thinking.
• Valid driver’s license and reliable transportation.
• Proficient with iPad/Laptop for visit notes and EMR.
• Bi-Lingual in Russian/English or Spanish/English is a plus.

Responsibilities

• Evaluate patients and set Start of Care treatment plans with physicians or orders.
• Assist patients with exercises to improve strength, coordination, and ROM.
• Teach adaptive equipment and assistive technology for daily tasks.
• Provide education to patients, families, and caregivers.
• Provide therapeutic treatment per physician orders to improve function.
• Coordinate care and discharge planning with patient, physicians and other disciplines.
• Collaborate with clinicians to provide comprehensive care.
• Maintain accurate patient records and documentation per regulations.
• Stay current with OT developments through continuing education.
