Job summary

PruittHealth is seeking a PRN Occupational Therapist (OT) for Home Health in North Carolina, with special consideration for Alamance or Guilford County residents. You will provide evaluative, consultative, and treatment interventions in patients’ homes, collaborating with the interdisciplinary team to develop and manage the care plan.

Must hold a valid, unrestricted state license and a ACOTE-accredited education; Home Health experience is preferred.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of ACOTE accredited school.
• If foreign-trained, program deemed substantially equivalent to US entry-level education by a credentials evaluation organization approved by AOTA.

Responsibilities

• Assesses, monitors, documents and reports patient health progress using required documents via clinician-preferred technology.
• Collaborates with interdisciplinary team to develop, integrate, and manage the plan of care and make referrals as needed.
• Demonstrates home care competency and knowledge for patient care skills.
• Completes timely documentation (e.g., OASIS, SOC/Admissions, Recertifications, ABNs, visits, orders, care coordination).
• Maintains compliance with safety regulations and agency guidelines.
• Meets productivity standards and participates in required in-services and team activities.
