Job summary

Community Health Resources, Inc. seeks a clinician for our Mobile OTP in Putnam, CT. The role delivers MOUD/Methadone services, with clinicians based at the Putnam OTP and occasionally serving in the community aboard the mobile unit.

RV coordination differs from driving the vehicle, with emphasis on direct patient care. You will perform assessments, risk evaluations, and develop person-centered treatment plans, working within a collaborative team to support clients and families while maintaining

Qualifications

• Master’s Degree or higher in related field.
• Experience working with trauma, mental illness and/or substance use disorders preferred.
• Licensure/Certification: LMSW, LPC-A, LMFT-A required; LCSW, LMFT, LPC, Licensed Psychologist preferred.

Responsibilities

• Provides assessment and treatment interventions consistent with the program model.
• Conducts clinical and risk assessments.
• Collaborates with individuals to create individualized, person-centered treatment plans.
• Demonstrates competency in age related or specialty issues and developmental needs for each population served.
• Maintains collaborative relationships with family members and other service providers.
• Provides crisis assessment and intervention to monitor status of clients in crisis.
• May provide care in office, home, residential, and/or community-based settings based on program model and setting.
• Responsible for maintaining client and staff records according to agency policy.
• Participates in weekly supervision, consultations, and meetings.
