Job summary

Genesis HealthCare is seeking a Physical Therapist Assistant for full-time outpatient rehabilitation in the South Windsor, CT area. The role focuses on delivering in-home physical therapy services to older adults under the supervision of a licensed Physical Therapist, with flexible scheduling across various settings.

Responsibilities include evaluating, treating, documenting, and collaborating with the rehab team to restore function and enhance quality of life.

Qualifications

• Must have a valid license in the state(s) of practice.
• Must have a valid driver's license or reliable transportation to and from patients' residences.

Responsibilities

• Provides outpatient rehabilitation services in an in-home setting for older adults.
• Evaluates and treats in the most functional environment which is the patient's home.
• Supervises or assists with tests, evaluations and procedures under the licensed Physical Therapist.
• Delivers individualized physical therapy treatments including therapeutic exercises, gait training, manual therapies and safety education.
• Maintains timely documentation and coordinates with the rehabilitation team.
