Job summary

Genesis HealthCare is seeking a Physical Therapist for Powerback Rehabilitation to deliver outpatient rehabilitation services in a home setting, working with older adults in their residences or in independent living facilities. The role focuses on evaluating, treating, and coordinating care to restore independence and mobility.

Responsibilities include developing individualized plans, collaborating with the rehab team, and educating patients and caregivers.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• They must have a Master's degree in Physical Therapy; or
• They must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or
• They must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
• Must have a valid driver's license or reliable, independent transportation to and from the patients' residence.

Responsibilities

• Supervision of Physical Therapy Assistants, temporary licensees, students and support personnel as necessary.
• Adherence to department and patient treatment schedule, timely clinical and billing documentation.
• Attends and contributes to patient care, staffing conferences and related meetings.
• Makes referrals to other disciplines and agencies to address patient needs.
• Consults with rehabilitation team to select appropriate therapeutic regimen.
• Adapts program and/or plan of care to patient needs.
• Promotes a safe environment and enforces elimination of hazards.
• Orders supplies and equipment as necessary.
• Cleans, maintains, and reports repair needs of equipment.
• Puts patient service first to ensure high-quality care.

Education

Master's degree in Physical Therapy

Master's degree in PT related field + Bachelor's in PT

Bachelor's degree in Physical Therapy

PT license in state(s) of practice

Driver's license / reliable transportation

5 years PT experience
