Job summary

You will deliver outpatient rehabilitation in home or facility settings under PT supervision, with a flexible schedule across various settings.

The role emphasizes patient-centered care, collaboration with the rehab team, and independent documentation and program delivery to improve daily function for older adults.

Qualifications

• Must hold a valid Physical Therapist Assistant license where practicing.
• Must have a valid driver’s license or reliable transportation to patient residences.

Responsibilities

• Attend and contribute to patient care conferences and required meetings as a representative of physical therapy.
• Maintain adherence to treatment schedules and documentation requirements.
• Collaborate with rehab team to implement patient programs and activities.
• Assist the Physical Therapist during tests and evaluations.
• Instruct patients and families in treatment techniques under supervision.
• Maintain timely documentation for all patients.
