Job summary

Genesis HealthCare is seeking a Physical Therapist Assistant for mobile outpatient therapy in Westfield, MA. The role supports ALF/ILF and community visits with a flexible schedule including full-time and part-time options.

Responsibilities include delivering outpatient rehabilitation to older adults in-home or in living facilities, under supervision of a licensed Physical Therapist, with emphasis on functional independence and patient-centered care.

Qualifications

• Must have a valid license in the state(s) of practice.
• Must have a valid driver's license or reliable transportation to and from patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Evaluate and treat in the most functional environment the patient's home.
• Maintain documentation of treatments and communicate with the supervising Physical Therapist.
• Instruct patients and caregivers in home-based therapy techniques.
• Ensure patient safety and adapt interventions to each patient’s needs.
