Job summary

CVS Health, through Signify Health, seeks a Full Time Physician to conduct in-home health evaluations across assigned regions. You will travel to patient homes, perform medical reviews, and coordinate with health plans to connect patients with appropriate care.

This role emphasizes patient-centered care delivered in home settings. Candidates must hold an active medical license in the coverage area and be able to obtain/maintain multi-state licenses.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Ability to obtain and maintain multi-state licenses as required.

Responsibilities

• Provide in-home health evaluations for patients and families.
• Travel up to 100% across defined coverage areas; may require overnight stays.
• Engage with health plans to ensure appropriate care connections.
• Use digital workflow tools during visits and document outcomes.
• Coordinate with care teams to support patient health goals.
