Job summary

CVS Health, through Signify Health, is seeking a Full Time Physician to provide in-home health evaluations. You will assess patients in their homes, review medical history, and perform physical evaluations while coordinating care with health plans and care teams.

The role supports nationwide visits and requires travel within licensed states. The position offers comprehensive benefits, a fixed 40-hour week, and opportunities to participate in a growing network dedicated to bridging gaps in care

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).
