Job summary

Powerback is seeking a Physical Therapist for full-time mobile outpatient therapy serving Westfield, Monson, and Holyoke. The role focuses on in-home outpatient rehabilitation for older adults, delivering personalized programs in patients' homes or living facilities.

You will collaborate with the rehab team, document care, adapt plans, supervise aides as needed, and participate in mentorship and CEU opportunities.

Qualifications

• Must have a valid Physical Therapy license in state(s) of practice, or proof of license eligibility.
• Master's degree in Physical Therapy is required or equivalent.
• Bachelor's degree in Physical Therapy with 5 years of progressive PT experience acceptable.

Responsibilities

• Provide outpatient rehabilitation services in patients' homes or living facilities.
• Develop and deliver individualized physical therapy programs to restore and enhance function.
• Document clinical and billing activities and participate in team meetings.
• Supervise PT assistants and students as required by licensure.
• Refer to other disciplines when needed to address comprehensive needs.
• Adapt plans of care based on patient progress and goals.
