Job summary

Powerback is seeking a Physical Therapist to deliver high-quality outpatient rehabilitation in the patient’s home and in assisted living settings. You will evaluate, plan, and implement individualized therapy programs for older adults, promoting independence and functional recovery across diverse environments.

Responsibilities include coordinating with care teams, documenting progress, and providing education to patients and caregivers.

Qualifications

• Must have a valid Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy or related field with a PT degree combination.
• Bachelor's degree in Physical Therapy with 5 years of relevant experience is acceptable.
• Driver's license or reliable transportation to patients' residences.
• Experience delivering outpatient/home-based PT is desirable.

Responsibilities

• Provide in-home outpatient rehabilitation services to older adults.
• Develop and deliver individualized PT plans to restore function.
• Assess needs, refer to other disciplines as needed, and coordinate with caregivers.
• Educate patients and families to promote independence and safety at home.
• Maintain timely documentation for clinical and billing purposes.
• Ensure a safe, quality care environment during home visits.
