BronxWorks in Bronx, NY is seeking a Homeless Outreach Case Manager to independently manage a diverse caseload and conduct regular field visits with chronically homeless individuals. The role involves developing service plans, coordinating referrals for mental health, medical care, housing, and benefits, and documenting progress.

A Bachelor’s in social work or related field and a valid driver’s license are required.

Qualifications

• Bachelor’s degree in social work or a related field.
• Valid driver’s license held for at least 1 year; in last 3 years, no more than 3 license points.
• One year of case management or related work experience preferred.
• Ability to engage and build rapport with high-need populations.
• Strong oral and written communication skills.
• Detail-oriented with strong time management and organizational skills.
• Proficiency in Microsoft Office; bilingual/multilingual preferred but not required.

Responsabilités

• Independently manage a caseload.
• Conduct regular field visits to engage and build rapport with chronically homeless individuals.
• Work with clients to develop goals and service plans.
• Provide referrals and direct assistance for mental health, medical care, substance treatment, vital documents, benefits, legal/immigration services, and permanent housing.
• Document all interactions and monitor progress toward service plan goals.
• Maintain familiarity with social services and community resources.
• Coordinate client care with other service providers; attend regular case conferences as required.
• Attend regular supervision sessions with direct supervisor.
• Drive agency vehicle to and from client locations and appointments.
• Perform additional duties as assigned.
