Job summary
La Casa De Salud is seeking a Community Outreach Worker in Bronx, NY to deliver community-based services for homeless and vulnerable populations. The role includes engaging hard-to-reach individuals, supporting care navigation, and coordinating connections to medical, behavioral health, and social services.
You will also help drive and maintain our mobile health unit. The position requires outreach experience, a valid NYS driver's license, and the ability to work in diverse, outdoor/mobile
Qualifications
- High School Diploma or GED required; associate degree preferred.
- Comfort with using smartphones/tablets/laptops for data entry and communication.
- Understanding of trauma-informed practices and serving underserved communities.
Responsibilities
- Conduct targeted outreach in areas with high unmet needs including shelters and public spaces.
- Build rapport with individuals facing housing instability using trauma-informed, culturally responsive approaches.
- Provide information on LCDS services and connect clients to medical, behavioral health, and social supports.
- Facilitate referrals and warm hand-offs to health care, mental health, recovery, and housing services.
- Operate the mobile outreach vehicle and maintain safety/documentation for trips.
