Job summary

The University of Chicago Medicine's Chicago Center for HIV Elimination seeks a Mobile Service Care Coordinator to expand STI/HIV testing and linkage to care in South Side communities. You will manage a caseload, support retention in care, and follow fidelity monitoring for data collection.

Work involves phlebotomy, outreach for HIV/STI testing, PrEP services, and documenting encounters with consent and releases. You will collaborate with teams and connect clients to resources.

Qualifications

• Experience working with LGBT populations and communities of color.
• Ability to conduct STI/HIV testing and linkage to care.
• Willingness to travel to community venues.
• Clinical study data collection experience preferred.

Responsibilities

• Support drop-in and outreach STI/HIV testing services including phlebotomy and linkage to care.
• Support policy and protocol development for mobile HIV, STI, SUD and MH care.
• Document all testing encounters with consent, releases, risk-factor screening and counseling.
• Pre-screen individuals for research projects and supportive service needs.
• Assist with day-to-day Supportive Services tasks and rapid response support.
• Acquire and apply knowledge of clinical studies to collect data.
