Job summary

The University of Chicago Medicine’s Chicago Center for HIV Elimination seeks a Linkage to Care Coordinator specializing in Black gender and sexually diverse populations. You will conduct STI and HIV testing, manage a care-cascade caseload, and ensure fidelity in data collection for ongoing research projects.

This grant-funded role requires retention support, community outreach, and collaboration across teams, with flexibility for evenings and weekends as needed.

Qualifications

• Bachelor's or advanced degree in a relevant field.
• Experience working with LGBT populations and Black/Latino communities.
• Experience with HIV/STI treatment or prevention is preferred.

Responsibilities

• Support STI and HIV testing and linkage to care services.
• Document testing encounters with consent, releases and counseling notes.
• Pre-screen for research projects and supportive service needs.
• Support outreach projects and resource counseling for communities of color and LGBTQ populations.
• Provide telephone and in-person support, assessments and safety planning.
• Maintain documentation and quality assurance as required.
