Job summary

The University of Chicago Medicine's Chicago Center for HIV Elimination is seeking a Mobile Service Care Coordinator focused on Black gender and sexually diverse populations in Chicago. You will conduct STI and HIV testing, deliver linkage to care, and manage a caseload within research projects while documenting encounters and maintaining fidelity to study protocols.

This grant-funded role requires strong interpersonal and organizational skills, experience with Microsoft Office, and a commitment

Qualifications

Education: vocational training or equivalent experience; a four-year degree is not required.
Work experience: knowledge and skills developed through related roles.

Responsibilities

Support STI and HIV testing, linkage to care, treatment and PrEP services.
Document testing encounters and counseling, including consent and notes.
Pre-screen for research projects and supportive service needs.
Assist outreach projects across community venues and events.
Provide resource counseling and conduct referrals for people of color and LGBTQ+ communities.
Coordinate with teams and perform administrative/data tasks as needed.
