Job summary

Lake Superior Community Health Center, Inc. is seeking a Community Outreach RN to deliver direct clinical care and mobile outreach for people facing barriers to healthcare. The role focuses on trauma-informed care and connecting patients with primary care in both clinic and community settings.

Responsibilities include triage, wound care, vaccinations, phlebotomy, care coordination, telehealth support, and timely documentation in the EMR. Flexibility for evenings and weekend coverage is expected.

Qualifications

• Associate or bachelor’s degree in nursing from an accredited School of Nursing.
• Active and unrestricted RN license in Minnesota and Wisconsin.
• Ability to provide patient-centered, trauma-informed care.

Responsibilities

• Provide professional clinical care in fixed-site clinic and community settings.
• Deliver skilled interventions including wound care, phlebotomy, and vaccines.
• Coordinate care and assist in provider visits via telehealth.
• Support care transitions and linkages to primary care.
• Document encounters accurately in the EMR.
