Job summary

Lake Superior Community Health Center seeks an Outreach RN to provide direct clinical care and case management in LSCHC clinics and in the community, with a focus on trauma-informed care and harm-reduction approaches for unhoused populations.

Responsibilities include triage, wound and foot care, vaccines, phlebotomy, care coordination, telehealth support, and linking patients to primary care and social services, while ensuring accurate EMR documentation.

Qualifications

• Associate or bachelor’s degree in nursing from an accredited program.
• Active and unrestricted RN license in Minnesota and Wisconsin.
• Ability to provide care in a sensitive, nonjudgmental manner.

Responsibilities

• Provide professional clinical care in fixed-site clinics and community settings.
• Deliver wound care, vaccines, phlebotomy, and vital signs.
• Coordinate visits via telehealth or in the community.
• Utilize harm reduction strategies for unhoused populations.
