Job summary

Chi-Care Med seeks a Nurse (RN) to deliver frontline clinical care during mobile outreach across Chicago. You will perform acute urgent care, infectious disease screening, vaccinations, and harm reduction education while linking patients to ongoing care.

The role involves teamwork with clinical providers, Peacekeepers, and outreach staff in various outreach settings. A valid Illinois RN license and BLS/ACLS are required; travel to shelters and street sites is expected.

Qualifications

• Active Illinois RN license required.
• BLS/ACLS certification required.
• Knowledge of harm reduction and street-based medical care preferred.
• Strong interpersonal, teaching, and teamwork skills required.
• Willing to travel to outreach settings (shelters, street sites, van).

Responsibilities

• Coordinate patient triage, intake, and record creation.
• Record vital signs and basic physical assessments.
• Administer vaccinations and dispense meds as needed.
• Support clinical providers during street outreach and mobile van visits.
• Maintain inventory and manage supplies storage.
• Document in EHR per confidentiality guidelines.
• Identify social needs and connect to higher-level care.
• Collaborate with Peacekeepers and outreach staff for safe operations.
• Provide education to students, volunteers, or new staff.
• Participate in team trainings and meetings.
