Job summary

Lake Superior Community Health Center is seeking an Outreach Registered Nurse to provide direct clinical care and patient case management in LSCHC clinics and in the community. This mobile role focuses on trauma-informed care and linkage to primary care for individuals facing barriers to health, including homelessness and substance use.

Responsibilities include triage, wound care, phlebotomy, vaccines, care coordination, and documentation in the EMR.

Qualifications

• Associate or bachelor’s degree in nursing.
• Active and unrestricted RN license in Minnesota and Wisconsin.
• Public health nurse registration preferred.
• Clinical experience in community health and outreach-based nursing preferred.

Responsibilities

• Provide professional clinical care in fixed-site clinic and community settings.
• Deliver wound care, phlebotomy, vaccines, and vital signs.
• Coordinate and assist in provider visits through telehealth and in the community.
• Utilize harm reduction strategies for unhoused populations.
• Assist with care transitions and linkages to primary and specialty care.
• Document encounters in the EMR and communicate with the care team.
