Job summary

Lake County Health Department and Community Health Center is seeking a Community Health Worker to conduct outreach, engage patients in care management, and co-create individualized care plans using motivational interviewing.

You will work closely with the RN Care Manager and PCP, reviewing patient progress daily, and ensuring follow-up after hospitalization or ED visits, while documenting all interactions in the care team system.

Qualifications

• High School Diploma required.
• Experience in community work (advocacy, volunteering, or organizing) preferred.
• Case management experience preferred.
• Spanish or other language fluency preferred.
• Ability to work evenings and some weekends.
• Driver's license or ability to travel across the county.

Responsibilities

• Conduct outreach to patients and engage them in Complex Care Management.
• Co-create care plans with patients using motivational interviewing.
• Follow up after hospitalization or ED visits and track progress.
• Collaborate with RN Care Manager and PCP for reviews and escalations.
• Document each patient interaction in the care team records.
• Link patients to social services and entitlements (e.g., transportation).
• Assist patients in scheduling and accessing health services with their PCP.
• Maintain up-to-date records of care management activities.
