The Colorado Coalition for the Homeless is seeking a Health Outreach Program (HOP) Coordinator to supervise the mobile outreach, ensure safe operation of a 40 ft medical RV, manage sites, train staff, and foster community relations with Stout Street Health Center.

Responsibilities include client registration and navigation to services, plus potential Medical Assistant duties to deliver care across outreach and clinic settings. Bilingual English/Spanish skills are highly valued.

Qualifikationen

• High school diploma or equivalent required.
• Certification in Basic Life Support (CPR) required.
• EMT or Medical Assistant certification required.
• Must have and maintain a valid Colorado Driver’s License within 30 days of hire.

Aufgaben

• Safely drive the HOP mobile medical unit to service locations.
• Oversee setup and breakdown of the unit and maintain cleanliness.
• Oversee daily operations including scheduling and site coordination.
• Provide client registration, scheduling, and navigation to services.
• Record patient demographics, vitals, and visits in the EHR; support MA duties.
• Ensure OSHA/HIPAA compliance and maintain safety standards.
