La Casa de Salud in the Bronx, NY is seeking an Outreach Worker / Mobile Unit Driver to deliver community-based services to individuals experiencing homelessness and health challenges. You will engage hard-to-reach populations, support care navigation, and help connect clients to medical, behavioral health, and social services.

The role includes driving and maintaining a mobile health unit, coordinating referrals, scheduling, and ensuring confidentiality and safety.

Qualificações

High School Diploma or GED required; an associate degree is preferred.
Valid NYS Driver’s License with a clean driving record.
1+ year(s) of outreach, case management, peer support, or human services preferred.
Familiarity with underserved communities, harm reduction, and trauma-informed practices (training provided).
Comfortable engaging outdoors and in mobile settings under varying conditions.
Proficient with data entry on mobile devices (smartphones/tablets).

Responsabilidades

Perform targeted outreach in areas with unmet need to engage individuals.
Navigate services, set appointments, coordinate follow-ups, and document interactions.
Transport clients safely to medical, detox, housing, or social services as needed.
Operate and maintain the mobile outreach vehicle during scheduled hours.
Document outreach activities accurately and maintain client confidentiality.
